A Case of Intrapulmonary Solitary Fibrous Tumor

78歳，男性．前医での喘息の精査治療中，胸部CTにて左下葉に40mmの境界明瞭で辺縁整な充実性腫瘤を認めた．PET-CTでは腫瘤に一致したSUVmax 2.4のFDG集積を認めた．気管支鏡下生検では確定診断に至らなかったが，増大を示し，左下葉肺癌疑いにて診断治療目的に胸腔鏡下左肺下葉切除術および縦隔リンパ節郭清（ND2a-1）を施行した．切除標本割面の肉眼所見では臓側胸膜とは接しない境界明瞭な白色調の腫瘤を認め，病理組織検査では腫瘍は束状に錯綜する紡錘形細胞から構成され，免疫染色ではCD34（+），STAT6（+），S-100（-）を示し，肺内孤立性線維性腫瘍と診断した．腫瘍径は5cm以下で壊死像を認めず，核分裂像は高倍率10視野あたり（/10HPF）3個以下で悪性を示唆する所見は認めなかった．稀な肺内孤立性線維性腫瘍の1例を経験したため報告する．
